Although plaque psoriasis may appear in any part of the body, usually it is localized on extensor surfaces (sites of trauma) such as the elbows, knees, scalp and the lumbosacral region. In any case it is not excluded that this same type of psoriasis may involve other skin areas, such as the palm of the hands and the soles of the feet, the toenails, the fingernails or the genitals, so as to characterize a specific type of psoriasis for each different location.

 

The appearance of plaque is typically symmetric; the number of lesions can vary from a few isolated elements to numerous confluent lesions involving the whole skin (generalized psoriasis).

 

The lesions have four fundamental features:

- They are sharply demarcated patches.

- The surface is formed by non-cohesive silver scales.

- Underneath the scales of the skin, it is shown a bright and homogenous erythema.

- The sign of Auspitz is observed.

The lesions are of variable size from the size of the head of a pin to patches that cover large areas of the body. They persist for months or years and constantly produce a large amount of scales with little alteration in size or distribution of the individual patches. Occasionally the lesions converge and form plaques in which the edges resemble a map.

Causes

 

Recent research suggests that psoriasis is a disease probably related to abnormal immune system. Special blood cells that identify and destroy foreign material, such as viruses and bacteria, called white blood cells (or leukocytes) are part of the immune system. There are two types of white blood cells: T lymphocytes and B lymphocytes, when the T lymphocytes identify a substance or a foreign body it attacks it. When B cells identify a foreign body it produces instead special chemical substances called antibodies, these attach to the foreign body and destroy it. Psoriasis is associated with abnormal activity of the immune system, and especially of T-lymphocytes, as regards the skin. The T cells cause inflammation of the skin and cause skin cells to reproduce abnormally and excessively.

 

About one third of cases of psoriasis is hereditary. The researchers are studying families affected by psoriasis to identify the genes underlying the disease.

 

The cutaneous manifestation cyclically improves and then deteriorate again, among the conditions that can cause deterioration or triggering of the outbreaks are: 

  • Climate changes (usually the condition improves in summer and worsens in winter),
  • Physical traumas,
  • Infections (beta-hemolytic streptococcal infections, candida, HIV),
  • Stress,
  • Drugs (beta-blockers commonly used to treat high blood pressure such as propranolol atenolol, sotalol), lithium salts, antimalarials (such as chloroquine and hydroxychloroquine), NSAIDs, angiotensin inhibitors also used for hypertension.
  • Alcoholism,
  • Smoking.

The treatment of plaque psoriasis depends on the extent and severity of the condition, if it is a localized psoriasis, the treatment of choice is a topical treatment and in moderate and severe forms, systemic therapy (methotrexate, cyclosporine, retinoids and in refractory cases the biological therapy) is associated with a topical treatment).
