Guttate Psoriasis

 

Guttate Psoriasis represents less than 2% of cases and occurs more frequently in children and young adults. The skin appears covered with pink-red spots and lesions are less thick than plaque psoriasis.

 

 

Guttate Psoriasis represents less than 2% of cases and occurs more frequently in children and young adults, usually after an episode of pharyngitis, tonsillitis or otitis caused by a group A beta-hemolytic Streptococcus, with an appearance similar to small red drops covered by the usual silver-white scales, with a dimension that varies a few millimeters from 1.5 to 2 cm. Sometimes a disseminated macular rash may precede this type of psoriasis.

 

The skin appears covered with pink-red spots and lesions are less thick than plaque psoriasis. The areas where guttate psoriasis most often appears are the trunk, upper arms, thighs, and scalp.

 

This type of psoriasis can disappear spontaneously in a few weeks, but in cases in which the disease is persistent it can progress to plaque psoriasis and a controlled treatment in a doctor’s consultation is necessary.

                                                                                    Causes

 

The specific cause of guttate psoriasis is still unknown, but it is thought to be caused by an immune system deficiency that is greatly weakened after a strong infection, and is preceded in two thirds of cases by a pharyngeal infection caused by Streptococcus pyogenes (beta hemolytic streptococcus group A). In children it has been reported associated with infectious precipitants, such as chicken pox, rubella or measles, due to excessive emotional stress or emotional trauma. Lesions can come together and produce plaques but there may be spontaneous remission in weeks or months. Vulgar psoriasis starts as a variety of drops in 15% of cases.

 

This disorder of the skin, in fact, occurs at a young age when the changes, both psychically and physically, are multiple and sometimes challenging. This is a very sensitive age and it’s when the "immune identity" is built in view of the growth.

 

At a time when the stress does not subside and you are unable to regain your balance, the disease worsens. In periods of high stress, it is noticed some of the disease peaks, which increase its aggression. Once contracted the disease, it is hard to get out, it has an unpredictable development: in some periods is silent, in others more aggressive. In fact, the disease cannot be cured, but you can take action to prevent exacerbations and complications associated with it.

 

To really know that it is guttate psoriasis, you need to go to a dermatologist who will carry out the diagnosis using specific calculation indices such as the PASI (Psoriasis Area Severity Index), which is an index that quantifies the severity of psoriasis per affected area, which basically says what stage the disease is. Thereafter, the doctor may advise you to perform a tissue biopsy to corroborate the disease and establish a targeted treatment to try to alleviate the associated disorders.

You do not recover from guttate psoriasis. The proposed treatments by a specialist are remedies that can relieve the itching and the skin inflammation, but it’s good to start right away to get used to them.

 

The treatments are mainly topical. They involve the use of ointments, creams, foam or unguents applied to the skin, or the phototherapy (the use of UV rays). For psoriasis in general, and also for guttate psoriasis, solar exposure is very effective because it can alleviate the disorder and ease the manifestations by protecting us properly and preventing sunburn. For that reason, it’s important to use a sunscreen or sun block with a very high sun protection factor, paying attention to the hours of exposure to sunlight, and taking care of washing the skin very often without heating it up too much, to prevent the worsening of the disease.

 

The ointments are usually formulated with cortisone, which have the function of reducing the local immune response and thus the itching, but these cannot be used for long periods and may cause side effects.

 

There are reports of cases receiving treatment with systemic antibiotics, including erythromycin, amoxicillin, clavulanic acid, penicillin and rifampicin, with variable results. The efficacy of the use of antibiotics in guttate psoriasis in children remains controversial.

 

There are not enough data for the recommendation on tonsillectomy in patients with guttate psoriasis, its benefits are still a much-discussed issue.

Carefully clean the parts of the skin to which you will apply the cream (we recommend using hydrogen peroxide at 3% or 10 volumes - normal, commonly available hydrogen peroxide) by gently patting the skin to eliminate superficial scales. Wait until the area is completely dry before applying an appropriate amount of cream to the affected area.

Psoriasis with large and thick scales

After using the body wash, carefully clean the parts of the skin to which you will apply the cream (we recommend using hydrogen peroxide at 3% or 10 volumes - normal, commonly available hydrogen peroxide) by gently patting the skin to eliminate superficial scales. Wait until the area is completely dry before applying an appropriate amount of cream to the affected area.

Use Psotherapy™ Body Wash

For palmoplantar psoriasis, dispense three times the Body Wash in about 5 liters of water and perform handbaths or footbaths twice a day for 15 minutes each. Dry hands and feet carefully, avoiding rubbing strongly, using the utmost delicacy in irritated areas.

Wait until the area is completely dry before applying an appropriate amount of cream to the affected area by gently dabbing it on with your fingers or your hand; never massage it in strongly. Apply a thin layer of the product to the skin and let the cream completely air dry.

SCALP PSORIASIS

Use Psotherapy™ Scalp Fluid

Apply the right dose on affected areas of the scalp or on those hard to reach, such as those covered by hair, the night before hair washing. The next morning, wash your hair with Psotherapy® Shampoo. Rub gently the skin especially on affected areas.

Use Psotherapy™ Shampoo

Apply the right amount of Psotherapy® Shampoo to dry scalp and let it work for at least 4 minutes, then rinse with warm water. If you prefer, you can repeat the shampoo application by decreasing the action time to 1 minute. It is advised to wash the hair again if you are using Psotherapy® Scalp Fluid.  Psotherapy® shampoo does not generate a lot of foam, so do not overdo the amount in order to avoid unnecessary waste.    

 Use a narrow-toothed comb to remove any flaking skin residues. Avoid the comb from touching the scalp.

Repeat scalp wash at least three times a week. Apply scalp fluid in the evening, the shampoo in the morning, until the symptoms disappear, then once a week

NAIL PSORIASIS

Use Psotherapy™ Cream

Our cream is also effective against nail psoriasis. The area to be treated is definitely trickier than skin areas and it requires more effort and patience. Before applying our cream, you should pre-treat the nails with a fine-grain nail file to remove the surface layer that is normally covered with grease and dirt.

 Since nail tissue is extremely porous, it tends to absorb a lot of grease and impurities that create a barrier that would slow down or even prevent the absorption of our active ingredient. I advise you not to overdo the abrasion; just very light abrasion to remove a very thin surface layer will be enough. After that, cleanse your nails with hydrogen peroxide and when they are perfectly dry, apply our cream. We suggest that you apply an occlusive dressing (with special finger stalls sold in pharmacies or just simple cellophane) so the product will be left in place for longer time. The occlusive dressing is not mandatory but we recommend it if you perform activities with your hands or if you have to frequently wash your hands.

PSORIASIS ON PRIVATE PARTS

 Use Psotherapy™ Scalp Fluid

Apply the right dose of product on affected areas, even inside the genital area, the night before washing your private parts. The next morning, wash your genital area with Psotherapy® Shampoo. Rub gently the skin especially on affected areas.

Apply the right amount of Psotherapy® Shampoo to dry genital parts and let it work for at least 4 minutes, then rinse with warm water. Repeat the shampoo application by decreasing the action time to 1 minute. It is advised a second washing if you are using Psotherapy® Scalp Fluid.  Psotherapy® shampoo does not generate a lot of foam, so do not overdo the amount in order to avoid unnecessary waste.

Paradoxical Effect

It is important to know that at the beginning of using the product might appear redness and, sometimes, the itching increase. This is normal during the first two weeks of use; in fact, you may experience some symptoms known as "paradoxical symptoms", which in fact consist of an increased feeling of itching, redness of the lesions, and sometimes a more or less intense burning sensation.

These symptoms are normal and evidence that the active ingredient of Psotherapy is binding with the epidermal receptors in an attempt to soothe the inflammation.

 Obviously, the more the receptors are damaged, the more intense these symptoms will be; in any case, they will improve in a few days. You have to be patient and make a small sacrifice, since when these symptoms are gone, the receptors will start to regenerate and the symptoms of psoriasis will start to disappear.

Shampoo and Body Wash

The cleansing bases in the body wash ensure a proper cleaning. These products should replace the usual personal hygiene products during treatment. Since Psotherapy® Shampoo and Body Wash are extremely delicate they can be use frequently, even daily. Psotherapy® Shampoo and Body Wash do not generate a lot of foam, so do not overdo the amount in order to avoid unnecessary waste.

Drugs

Before starting Psotherapy treatment, it is advised to stop using cortisone products at least two weeks before and at least two months before for biological drugs.

It’s important to know the interaction with other medications, because, although they are not for psoriasis, they can delay or block the effect mechanism of Psotherapy products.
